Hand this packet to the IC at the start of the session. One sheet per round – fill it in after each team’s brief, before announcing a decision. Collect all 5 sheets at session end. The completed packet is your primary debrief artifact: you can ask the IC to read back what they wrote in Round 2 and the room can assess whether that synthesis was accurate.
